Be sure to complete all required fields marked with a star on the forms.
BEFORE YOU BEGIN | Be prepared to upload a photo of your Florida drivers license and your Medical Marijuana Card. (The MM Card is uploaded in place of proof of condition.)

After your forms are submitted, a Schedule Appointment button will appear on the same page.
RECERTIFICATIONS | Select one of the locations designated as ‘Recertifications‘ and then the service designated as ‘Existing Patient‘.
Be sure to review your appointment confirmation email and instructions.
