If you’ve been diagnosed by a doctor with a condition that makes you eligible to receive a medical card, such as any condition listed in our Eligibility Section, you need to upload this along with your driver’s license.
Accepted file formats are: jpg, gif, png.
If you have been diagnosed with a condition that you require medical cannabis for, such as any condition listed in our Eligibility section, you should be able to get a record of this from your doctor. This counts as your “Proof of Condition”.

IMPORTANT TO KNOW
You are required to have a qualifying condition to be approved for medical marijuana. Medical marijuana doctors cannot diagnose and recommend patients for medical marijuana.
CANCELLATION POLICY
We are committed to providing exceptional care. Unfortunately, when one patient cancels without giving enough notice, they prevent another patient from being seen. To avoid a $30 cancellation fee, just message us at 813.774.3474 before 10:00 am the day prior to your appointment or the Friday before if your appointment is on Monday.
DISCLAIMER
By submitting and signing our Patient Intake forms you acknowledge you are a patient of Medical Marijuana Clinic, not a patient of the physician you will see at your scheduled appointment. If at any time that physician is no longer associated with Medical Marijuana Clinic, you will be activated in the Medical Marijuana Clinic’s medical director’s registry account.
PATIENT’S TRANSFERRING FROM OTHER CLINICS
If you are registered in the MMUR and transferring your care to our clinic, when scheduling your appointment, select one of the locations listed as ‘New Patient’, then select ‘Transfer’ service.
Before our office can enter your new certification and orders, you must deactivate your registry account from your previous physician in the MMU Registry account. Follow this link for instructions.

The Process is super easy. Most patients say after their appointment, “Is that it? If I would have known it was that easy, I would have done it much sooner!”
You will receive your medical marijuana card approval after you submit your application to the State. If you are a Florida Resident, approvals are typically within 24-48 hours of submitting your application. If there is a problem with your application or you are a Seasonal Resident, it could take about 2-3 business days to receive your approval. For more details, keep reading!

How long does it take to get approved for a medical marijuana card?
The state has drastically shortened the approval period and is efficiently processing applications within 24 to 48 hours if you are a Florida Resident. Seasonal Residents, or application issues, can usually receive approval within 3 to 5 days.
Why only a 7-month period?
Florida law requires a new certification and orders every 210 days, this means you must get re-certified every 7 months. But don’t worry – all you must do is schedule your recertification appointment online.
Why are medical marijuana cards good for 12-months but my certification is only good for 7-months?
We aren’t really sure why the State set it up like that, but they probably had a good reason. We will remind you in about 6-months from your doctor’s appointment that it is time to recertify. The State will send you a reminder to renew your card. Be sure to keep an eye out for those email reminders!
Can I just visit my regular doctor, do I still need to use your service?
Florida law also requires you to be seen by a certified medical marijuana doctor to obtain their approval for certification and medical marijuana orders. Once you have proof of condition from your doctor, follow our easy 3-step process to receive your medical marijuana card as quickly as possible.
